  • SECTION FOUR: PROGRAM RULES AND RESPONSIBILITIES

    I understand that registering for the NAFTZ Mentoring Program does not necessarily guarantee participation in the mentoring program if NAFTZ cannot make a suitable mentee match. While NAFTZ will work to match me with a suitable mentee for the duration of the program, it cannot guarantee me a mentee. The program is designed to be at least a 12 month commitment. I am able to attend quarterly meetings either by phone or in-person during the duration of the program unless my mentee and I choose an alternative meeting schedule. I understand that the mentee must take ownership and work with me in the arrangement of meeting times and locations. By signing below, you attest to the truthfulness of all information listed on this application. I have read and understood the program’s rules and responsibilities for becoming a mentor. If selected, I will follow the rules of the program and be a dedicated mentor. Any and all information submitted will be treated as confidential and will not be used other than to determine a strong match between mentor and mentee.
