Any and all information submitted will be treated as confidential and will not be used other than to determine a strong match between mentor and mentee. Program Checklist: I understand that registering for the NAFTZ Mentoring Program does not necessarily guarantee participation in the mentoring program if the NAFTZ cannot make a suitable mentor match. While NAFTZ will work to match me with a suitable mentor for the duration of the program, it cannot guarantee me a mentor. The program is designed to be at least a 12 month commitment. I am able to attend quarterly meetings either by phone or in-person during the duration of the program unless my mentor and I choose an alternative meeting schedule. I understand that my mentor has volunteered for the Program and that the relationship is learning based. The NAFTZ Mentoring Program is not an employment placement program. I understand that I must take ownership in the arrangement of meeting times and locations and work with my mentor’s schedule.
